Remove the success checkmark from tool call rows (#2659)
* drop the success checkmark from tool call rows
A finished tool call goes back to showing no icon: "Called tool" already
says it succeeded, and a check on every row added noise to long tool runs.
Failed calls keep the amber warning, including results that come back with
an error status.
* trim the comment and test changes to the minimum
