fix: restore MCP UI integration lost during merge
- Add scheme_host_port overload for server_http_proxy (MCP HTTPS support)
- Add ChatFormActionMcp button back to ChatFormActions
- Re-integrate MCP tool calling in chat store (collectMcpTools, executeToolCalls)
