Additional tests and a fix (#746)
- Test secondary commissioning of ServerNode
- Test subscriptions to attributes and events in new API
- Expose ExchangeManager in ServerNode environment for testing purposes
- Fix subscriptions to fabric-scoped attributes when session has no registered fabric
- Change "leave" event to trigger when fabric is removed via any source
