SILInliner: Fix scope & source location of terminator instructions. Instead of cloning existing instructions (and correctly remapping their scopes) SILCloner just creates a fresh instruction for the terminator and forgets to set the scope accordingly. This bug flew under the radar because the SIL verifier for continiuous scopes doesn't look at terminator instructions. rdar://97617367
