docs: link first App Shell mention to glossary
The four guides/API surfaces that introduce 'App Shell' now link the
first mention to /docs/app/glossary#app-shell. Later in-doc mentions
that point at the runtime-prefetching deep-dive section (#app-shells)
are unchanged.
